<胎兒先天性心臟病 - #大動脈轉位>
在胎兒先天性心臟病當中,有幾個是出生後就需要緊急介入的,大動脈轉位就是其中一種。
❤️在正常的心臟當中,心房心室的連接如下:
✔️右心房 -> 右心室 -> 肺動脈
✔️左心房 -> 左心室 -> 主動脈
而所謂的大動脈轉位,是寶寶在胚胎時期conotruncus發育出現問題,使得心室連接的大動脈交換了,缺氧血和充氧血交換出問題,導致寶寶出生後會發紺和呼吸急促。

1️⃣最基本的就是要判斷 心房 - 心室 - 大動脈 的連接是正確的,再來左右也要區分清楚。
✏️如何判斷左右心房心室在文章中有詳細介紹
✏️TGA的4CV常是正常的,因此無法以單一切面來診斷
✏️PA/Ao常是平行的
2️⃣判斷是哪種TGA?
TGA依照心房心室連接分成兩種: D-TGA, L-TGA(CCTGA)。
✏️D-loop ventricle代表我們A-V連接是正確的(AV concordance),右心房->右心室,右心室在心臟的右邊。
✏️L-loop ventricle則表示我們A-V連接相反(AV discordance),右心房->左心室,左心房->右心室。
3️⃣有沒有合併其他心臟的異常?
最常合併的異常: #VSD, #PulmonaryStenosis
4️⃣是否需要產前遺傳診斷?
✏️單純TGA大部分和染色體異常較無相關,但如果有合併其他器官異常,就強烈建議要做染色體檢驗。
✏️TBX1 gene被認為和conotruncal defects相關 (TBX1 gene loss function有可能是造成conotruncal defect的原因之一)

15. 第一部份,我的求生指南。就係我43年嘅故事。你心諗關我春事?我嚟求財的。但,你唔知我經歷,就唔知點解我今日有咁嘅思想。當然你買咗返去唔睇亦得,大拖羅嘛。入面仲會講我小小人生法則,點樣6年加咗4倍人工。仲有啲讀者問我入行呀,平時睇乜書乜文呀,甚至創作心得都會有講下。雖則唔覺得我有乜咁巴閉,但話晒都出到本書,話晒Facebook 都有20000人, Patreon 都有1800 人,都唔係個個得。
16. 第二部份,職場求生指南。應該唔使點解啦?教你點上位,點轉工,點跳草裙舞,揀入邊行,揀咩工做,有乜要留意,點揀個好老細。雖然講到尾,我覺得爬Corporate ladder 呢個遊戲已經玩完。但呢部份都係好重要,你以為我啲錢點嚟?除非你有父幹或者畀人包(我冇),否則,啲錢唔係打工嚟可以點嚟?咁冇錢,,投乜資?你十皮嘢我畀你贏十倍又點?
17. 第三部份,股市求生指南。冇教你睇圖睇年報,冇交易系統。好心啦,居然有人覺得百幾蚊有必勝法。主要講心法嘢,改變你自己。但我亦都有講一個「全天候投資法」,萬用理財方案。
18. 講到尾係你自己,投資法則網上一大堆啦,有啲好似矛盾,又好似好難執行。係冇得速成嘅,只有倉頡,速成害咗你。等於啲餐廳煮嘢咁,傳統做法工序多,你要速成一樣煮到丫,但做唔到人地嘅味道咯。等於你睇戲咁,你可以睇谷阿莫,好多戲嘅情況都係幾分鐘講完,但你唔由頭睇到尾,冇浸落去,就係唔同的。學藝不精嘅,我畀你好彩贏到幾倍,都一定輸返。除非你個本夠大,中完一舖夠勁嘅即時退休—但可能你再搞其他嘢都會出事,大把故事啲窮人中完六合彩好快又破產啦。
