【家庭醫學】~ 冰淇淋頭痛(ice-cream headache)
炎炎夏日,吃冰或喝杯冰涼的飲料,應該是很多人消暑的選擇。最近在門診聊天時,有些人反應到,某次剛吃完一大碗刨冰,或是從外面很熱回家,灌了一大杯冰水後,突然頭就爆痛,想衝出門到診所就醫時,又好像沒那麼不舒服了?
這其實就是本次標題說的:「冰淇淋頭痛」(ice-cream headache),或是「大腦凍結」(brain freeze)、「冷刺激頭痛」(cold-stimulus headache)。在醫學上的專有名稱是「蝶腭神經節疼痛」(sphenopalatine ganglioneuralgia)
〖成因〗
冰冷的食物或飲料,觸及到人的上腭時,人的神經反射會使血管急速收縮擴張;附近的疼痛感受器會感知這種擴張,然後通過「三叉神經」,將信號發送回大腦。
這種神經也能感知面部疼痛(大家應該聽過三叉神經痛吧?),因此在傳導神經信號時,大腦會將疼痛解釋為來自前額導致疼痛牽涉到上腭,並傳導到大腦。(這種稱為牽涉痛『referred pain』,比較常聽到的例子是有些人心肌梗塞時,卻是肩膀痛)
食用冰冷食物的頻率越高,冰淇淋頭痛發生的機率越高。
〖治療方法〗
就像前面的例子所說,這種痛,來得快,去得也快;當你下定決定出門找醫師時,往往疼痛就已經緩解了。(所以也不太需要治療啦)
不過如果痛得受不了,也可以使用舌頭頂住上腭,讓他快點回暖,也會有幫助唷(恭喜大家又學到一個沒什麼用的冷知識。)
但是,如果頭痛持續的時間很久,或是發作太頻繁,連沒吃冰冷的東西都會發作,就建議進一步到神經內科門診評估囉。

In 2015, we did a scan in Singapore after experiencing a sharp pain in my lower abdomen, accompanied by bloated stomach and loss of appetite for the past weeks. X-ray result showed a mass in my body. We decided to buy the earliest flight home (to Malaysia) the next day to seek for second opinion. It’s a family doctor, he too said the same thing and referred me to the specialist in General Hospital. While waiting for my surgery date, my stomach has gradually grown in size; almost like a pregnant woman, but the speed was faster. To the size of a 7-8 months pregnant tummy in less than 2 months time. Even though it didn’t happen by choice, but we were somehow excited too because we thought to ourselves, what if it’s a real baby inside? 😜 Everyone around us were worried and anxious about what this may be and how this will affect us and my health.. but both of us, we were just having fun time posing for pictures just Incase we don’t have this chance anymore. 【ℜ𝔢𝔣𝔢𝔯 𝔱𝔬 𝔱𝔥𝔢 𝔩𝔞𝔰𝔱 𝔭𝔦𝔠𝔱𝔲𝔯𝔢】It’s like either... I didn’t survive this or I can no longer conceive or we are no longer together. So living in the moment is the best thing to do especially in time of crisis! 😆💪🏼
Fast forward ⏩ , 6 years later. Thank you to the pandemic, we thought this is the best time we try if anything is possible and finally got pregnant. 🤰🏻 We were over the moon, absolutely excited and bursting with pride, that conceiving is possible afterall! 😝 Bae is still the same man who never stop assuring me that everything will be fine, cares for my well-being, rubbed my big tummy from time to time to ease my over-stretched/ hard/ tight tummy (esp after meals) and never once complain how much weight I’ve gained since 🙈🙉🙊 I’m just so blessed ❤️
On a side note, if the world is still as before without the lockdown n pandemic, and if I were to make a decision whether this is the right time to settle down.. I doubt I’ll say yes 🤣 𝘽𝙚𝙘𝙖𝙪𝙨𝙚 𝙞𝙩’𝙨 𝙖𝙡𝙬𝙖𝙮𝙨 𝙣𝙚𝙫𝙚𝙧 𝙖 𝙧𝙞𝙜𝙝𝙩 𝙩𝙞𝙢𝙚 𝙛𝙤𝙧 𝙖𝙣𝙮𝙩𝙝𝙞𝙣𝙜 𝙪𝙣𝙩𝙞𝙡 𝙩𝙝𝙚 𝙩𝙞𝙢𝙚 𝙝𝙖𝙨 𝙥𝙖𝙨𝙨𝙚𝙙 𝙖𝙣𝙙 𝙄 𝙬𝙞𝙡𝙡 𝙥𝙧𝙤𝙗𝙖𝙗𝙡𝙮 𝙧𝙚𝙜𝙧𝙚𝙩 𝙛𝙤𝙧 𝙣𝙤𝙩 𝙙𝙤𝙞𝙣𝙜 𝙞𝙩 𝙖𝙣𝙮 𝙚𝙖𝙧𝙡𝙞𝙚𝙧. Just glad for everything that happened, and I believe things always happened for a reason, I just need to be patient to see and understand what’s the outcome like. I am quite sure pregnancy-confinement-baby is not an easy journey, but we will try our best to fulfill our responsibilities as parents n guardian 🙌🏼

Some takeaways after being a mom for the first few weeks! Hopefully it’d be useful for other first time moms!
Postpartum outfit: Most of the time u’ll be 衣衫不整,頭髮凌亂,like a 癲婆🤣 All u want is comfortable PJs to dress in all day, something u can easily unbutton to breastfeed. Honestly, really appreciate the @tanicomfort Silktouch collection. All I can say is, get a few pairs, cuz u’ll quickly realize how all your clothes just get stained with milk, 🤣 comes in handy when u have a few pairs to change in!
Newborn outfit: Newborn babies have such sharp nails!! And they always want to scratch their faces! Or 亂咁揮拳,然後”wehh” 到塊面!So buy newborn outfits that wrap their hands!!! Cuz hand mittens easily come off, 包手嘅衫係最好㗎 for the first month!!
Breastfeeding: first few days I had no idea how breastfeeding worked as a first time mom ➡️ The paediatrician referred this lady to come teach us what to do. Oh God, she hand expressed my colostrum and it was sooooo F-ing painful, 真係人生最痛. But I think the most stressful thing wasn’t the physical pain, but the conflicting advices given by different people! Different people wholly confident with their ways, telling u what u should do! 🤯🤯 Now I know, just follow your heart, and follow the advice that stresses u the least!!! I realize it was not necessary to go thru sooo much of that hand expressing pain, neither was it necessary to go by the extremely strict schedule imposed by that lady who achieved nothing except stressed the sh*t outta me. So yes, if u want to avoid that person, please PM me.
Motherhood is not easy, there are so many decisions that needs to be made. All I can say is, surround yourself with supportive people, and cut out everything else that is negative and stressful. Focus on your own, your child, your husband’s well-being; and that’s enough. If something/someone doesn’t help you feel better or more relaxed, then they’re really not needed at the current stage you’re in!
